Refer someone for support

Home & Community Support accepts referrals for NDIS participants, DVA recipients and My Aged Care clients who need practical support at home, in the community or through respite.

Support that is planned around the person

When you refer someone to Home & Community Support, we take the time to understand what they need, what is already working and where extra help may be needed. 

Support may include personal care, domestic assistance, daily living support, social support, light gardening for existing clients, in-home respite or rural property respite near Edithburgh. We look at the person’s routine, comfort level, location and preferences so we can consider whether our team is the right fit before support begins.

We will not force a fit

We know referrals often come with moving parts, including funding, family input, existing providers, worker availability and changing support needs. Our approach is to communicate clearly from the beginning, including what we can provide, what availability may look like and whether we believe our team is suited to the person being referred. 

If we are not the right option, we will be honest about that and, where possible, help connect you with another provider who may be better placed to assist.
